Tc-99m-HIDA
hepatobiliäre Leberfunktionsszintigraphie
|
Indikation
|
- Leberrundherde ( FNH, Hepatom )
- Lebertransplantatnachsorge
- Frühformen des Verschlußikterus
- DD neonatale Ikterus-Hepatitis
- DD funktionelle-organische Gallenabflußstörung
|
Radio-
pharmakon |
Tc-99m-Bridatec / BRIDA / HIDA
HIDA = N-[2,6-Diäthylacetanilido-Iminodiacetat] = Derivat des Lidocains.
|
Aktivität |
100 - 185 MBq pro Patient in weniger als 1 ml (Bolus-Injektion)
oder 1,11 MBq / kg ( bei hohem Bilirubinwert ( d. h. > 5mg% ) => mehr Aktivität bis zu 400 MBq pro Patient )
|
Prinzip |
- Perfusionsphase (- 2min p.i.) => arterioportale Durchblutungsrelation (OP_Planung bei portokavalem SHUNT)
- Parenchymphase (-20min p.i.) :
- Raffungsmaximum norm 7-13 min p.i.
- Exkretionsphase norm 6-14 min p.i. „Erstes erscheinen im Ductus choledochus“
- norm Gallenblase 10-20 min p.i
- norm Duodenum 12-28 min p.i.
Anreicherung des Radiopharmakons in den Hepatocyten;
die Ausscheidung erfolgt über die Gallenblase. Nach etwa 60min sollte das Leberparenchym weitgehend frei von HIDA sein.
